Website privacy

This page explains what information Brightwater Family Medicine collects through this website and what we do with it. How we look after your medical records is explained in our Notice of Privacy Practices, which you can ask for at the front desk.

What we collect

When you send the appointment request form, we receive your name, phone number, email address if you give it, the type of visit and the days and times that suit you. We use them only to call you back and book your visit.

Please don’t send symptoms or other medical details through the form. Share those with us by phone, through the patient portal or at your visit.

Our host may keep standard server logs, such as IP addresses and the pages requested, to keep the site running and secure.

Cookies

This site does not set tracking or advertising cookies. If you choose “Larger text”, your browser remembers that choice on your device only.

Who we share it with

We do not sell your information. Form messages are stored by our hosting provider, which processes them on our behalf, and are deleted once your visit is booked.

Your choices

You can ask us to see, correct or delete the information we hold about you. Email hello@example.com or call (608) 555-0186, and we will reply within 30 days.
